Nothing here invents a rule. OPD registration at a government hospital is already free, 108 is already the emergency number, and hospitals already keep every one of these records. Sehat only changes who can see them, and when.

One record, four people around it.

The patient books and waits. The desk registers arrivals. The doctor consults and prescribes. The administrator watches the load across every clinic. All four read and write the same rows.

    Describes what is wrong, picks from slots that genuinely have room, and watches their token move while they wait. Afterwards the diagnosis, the medicines and any follow-up sit in their record permanently.

    Marks patients arrived as they reach the window, then sends them through to the room. Arrival is recorded separately from booking, which is what makes walk-ins and no-shows countable rather than guessed at.

    Opens the next patient with their previous visits and uploaded documents already on screen, records findings and diagnosis, prescribes by line, and sets a follow-up that books itself into the next free clinic.

A prescription is data, not a photograph.

The doctor enters each medicine as its own line — drug, dose, the 1-0-1 schedule, how many days. That is why the patient's phone can show a real timetable, why the next doctor can see what was already tried, and why “come back in two weeks” can book itself instead of ending up on a slip of paper that gets lost.

Patients can attach the paper prescriptions they already carry. Those land against the same hospital number.
